Developmental Disability Services

SCO Family of Services helps people with developmental disabilities realize their full potential and become participating members of their community.

A variety of residences and services are structured to allow children and adults with developmental disabilities to strive to become as independent as possible.

Opportunities at Home

SCO offers a range of services for people with developmental disabilities who live at home. A service plan is developed for each person according to their individual needs in order to help them attain their personal goals. These services are available as long as they meet eligibility requirements. (Note: Some of these programs are services offered under the Home and Community Based Waiver.)

Medicaid Service Coordination

The Medicaid Service Coordination program provides an array of services that give participants the chance to strive to reach goals they never before thought possible.

Non-Medicaid Service Coordination

SCO assists developmentally disabled children and adults in crisis who do not have Medicaid become eligible for Medicaid, Social Security Insurance and other services. We help those in dire circumstances get the assistance they need quickly so they can live safer, more stable lives.

In-Home Residential Habilitation

The Residential Habilitation program teaches children and adults who are living at home daily living skills and gives them the opportunity to participate in activities that foster independence and will allow them live in the community successfully.

In Home Respite

This service provides relief to the families caring for someone with a developmental disability. A respite worker cares for the child or adult in their home so their family can take advantage of some time needed to attend to other issues.

Day Habilitation

The focus of this program is to give behaviorally challenged adults with developmental disabilities who live at home or in Individual Residential Alternatives the opportunity to make a more meaningful contribution to their community. They volunteer and participate in a variety of local activities and they participate in clubs and recreational activities at the Day Habilitation Center.

Family Care

The Family Care Program is for individuals who do not require placement in a residential facility but who cannot function in their own homes. Family Care places these individuals with specially trained families who offer a structured and stable home environment, along with the support, guidance, love and companionship a family can provide. Family Care assists these families with agency administered treatment and support services.

Drop-In Lounge

The Drop-In Lounge is a place where adults with developmental disabilities can socialize and participate in recreational activities with their peers. We offer many group activities such as music, art, games, poetry, pot luck dinners, literature and International Night celebrating cultural diversity. Participation is voluntary so people can choose to join in a group activity or just socialize with friends.

Family Support Services

Family Support Services helps the families of children with disabilities obtain OMRDD reimbursement to pay for respite, camp, and other services.

Individualized Support Services

Individualized Support Services financially supplements a person’s income through OMRDD to help them live in the community near friends and family with the support of Residential Habilitation, Day Habilitation or the Job Coaching Program. The goal is to help recipients live as independently as possible.

Residences

SCO operates residences and group homes for children and adults with developmental disabilities throughout New York City and Long Island. Residential programs are structured according to the level of care and supervision each person requires. The goal is the same for each…to provide care and support so that each person can become as independent as he or she can be.

St. Christopher’s Home

Located in Sea Cliff, St. Christopher’s Home is a Residential Treatment Center (RTC) that serves 73 children and adolescents with developmental disabilities. The RTC provides 24-hour nursing care, social services and a variety of therapies that teach the children daily living skills according to their abilities. Children also participate in recreational activities and team sports that give them the opportunity to compete, work together as a group and celebrate their accomplishments.

The Sea Cliff campus includes a main residential building, residential cottages, handicap accessible recreation facilities and the brand new Tyree Learning Center, a school specifically designed to accommodate the needs of the children who reside on the Sea Cliff campus.

Intermediate Care Facilities

SCO’s Intermediate Care Facilities (ICFs) are residences with round-the-clock care for individuals with developmental disabilities. A caring staff, including psychologists, nurses and social workers attend to their physical, emotional, and educational needs. The individuals enjoy a wide variety of recreational activities as well as speech and physical therapy. They are also trained in living skills, with the goal of making each person as independent as abilities permit.

Individualized Residential Alternatives

Individual Residential Alternatives (IRAs) are small group homes (6 or fewer residents) that focus on promoting independence and preparing young adults with developmental disabilities to become productive members of their communities. These programs provide a nurturing environment in a supervised, home-like setting that allows each individual to achieve his or her maximum potential. Individual Residential Alternatives are located in Brooklyn, Queens, and throughout Long Island.

Supportive IRAs

Supportive IRAs are 1 or 2-bed residences for high functioning adults who don’t require 24-hour supervision. Staff members assist residents with medical appointments and continue working with them to enhance their independent living skills. Residents often attend day programs or work in the community as well. Supportive IRAs are located throughout Long Island.

Schools

The Christopher School

Set to open in September 2010, The Christopher School is a 12-month therapeutic residential school that will serve 24 children with developmental disabilities who require 24-hour supervision. Located on the grounds of the Crossroads Campus in the Bronx, its proximity will allow the kids to maintain close contact with their families and communities.

The Christopher School will provide a safe, nurturing home environment as well as an integrated educational experience. The treatment plan for each child will be seamlessly integrated throughout their day. Moreover, when the child readches age 21, SCO will provide a smooth transition to an adult placement carefully chosen to meet the needs of each child.

Tyree Learning Center

The Tyree Learning Center is a New York State approved special education school that serves the 73 children and adolescents with multiple disabilities who reside at St. Christopher’s Home in Sea Cliff. The new school building, which opened in 2008, was designed specifically to accommodate the children’s needs. Small class sizes and student-centered instruction tailors each student’s education to their individual abilities. The Tyree Learning Center enhances the quality of the students’ lives by helping them achieve their highest level of independence.
